    • I understand that, to reserve space for a mission team to attend Words to Works Ministries, I must submit a completed Initial Deposit Form and a nonrefundable Initial Church Deposit. *

    • I understand that the nonrefundable Midterm Deposit, as described in the Leader's Packet, must be postmarked by the due date. I understand that a $10.00 per person late fee will be charged if the deposit is not postmarked by the due date. *

    • I understand that the nonrefundable Final Payment must be postmarked by the due date and will contain the balance for the trip. I understand that a $25.00 per person late fee will be charged if the payment is no postmarked by the due date. *

    • I understand that all payments are nonrefundable. *

    • I understand that I must have a group total by the Midterm Deposit. If I am leading a 24 Hour Mission Trip, I understand I must have a group total by the Final Payment. *

    • I understand that I may not reduce the group size after the Midterm Deposit (or Final Payment for a 24 Hour Mission Trip). *

    • I understand that I may increase my group size after the Midterm Deposit (or Final Payment for a 24 Hour Mission Trip) if Words to Works authorizes the increase and I submit payment immediately. *
